All patients potentially eligible for liver
a transplant are pre-evaluated during an Out-Patient Clinic (OPC) consult and,
if eligible, undergo a more in-depth evaluation. They then undergo a detailed
diagnostic pathway (clinical, lab, and instrumental tests). The degree and
the nature of the liver failure, along with the level of functioning of the
other organs, are accurately determined to assess the clinical conditions for
possible transplant eligibility.
During this stage, it is established whether the patient meets the criteria
for inclusion on the waiting list, according to which the patient must have:
irreversible acute or chronic liver disease
a disease which cannot be treated with traditional medical-surgical therapies
no contraindications to the transplant
psychological ability to accept the transplant (except for emergencies)
reasonable chance of surviving the transplant and enjoying significant advantages
in terms of quantity and quality of life
As soon as the pre-transplant evaluation is over, the clinical case is presented
to the medical faculty to determine whether the patient can be added to the
waiting list, or if refusal of his/her candidacy is justifiable.
Once in the waiting list, all liver transplant candidates are monitored by ISMETT's
team by means of periodic OPC check-ups, according to a customized protocol
based on each patient's needs.
In the periodic check-ups following the inclusion on the waiting list, the medical
team can decide to keep the patient temporarily from the active list because
of concomitant episodes (infections), or definitively, should permanent contraindications
to the transplant arise.
