You can support ISMETT with your 5×1000 contribution. Insert ISMETT’s tax code 04544550827 in the check box “Finanziamento della Ricerca Sanitaria” and your signature.

All those who submit an income tax statement (UNICO or 730 form) or receive the CUD (Certificazione Unica al Dipendente – Italian Wage & Tax Statement) form from their employer, have the opportunity to choose the beneficiary of their 5X1000 contribution.

Bring the number with you and give it to a CAF [Fiscal Assistance Center], or to your accountant.
