The program started in 1999 with the Living Donor Kidney Transplant Program, and in 2001, the Deceased Donor Kidney Transplant Program was instituted.

ISMETT is part of a project, promoted by the National Transplantation Center, to perform kidney transplantation in cross-over modality in order to allow for a greater number of living donor transplants despite HLA or blood type incompatibility between the donor and the recipient.

At ISMETT, there are the combined kidney-pancreas, kidney-liver and kidney-heart transplant programs.
In 2001, for the first time in Italy, a living-related kidney transplant was performed on an HIV-positive patient.
