Admission

Hospital Admission Documents

For admission to IRCCS ISMETT, the scheduled arrival time is communicated to the patient on the day before admission.

At the time of admission, it is essential to bring the following documentation:

  • Valid ID document.
  • Hospital admission request on a Regional Health Service (SSR) prescription form and hospital access form (Attachment E) , completed by the patient’s GP.
  • Health Insurance Card (Tessera Sanitaria).
  • Tax code.
  • Any valid exemption cards for medical conditions, income status, or disability.
  • A detailed list of all medications currently being taken.
  • Reports from previous medical visits or diagnostic tests.

In the event of a sudden unavailability of beds due to unforeseen and unavoidable circumstances (such as an urgent transplant), a scheduled admission may be canceled at short notice. Should this be the case, IRCCS ISMETT will make every effort to reschedule the admission as soon as possible.

If, however, the patient is unable to come to the hospital on the scheduled day and time, he or she will be placed back on the waiting list with the date of the original unattended appointment.

Following these instructions will make the admission process more efficient and ensure a smoother experience for everyone.
